The intent is for players to propose new spells themselves. As for the addition of spells into a spell book, paragraph 3, sentence 3 states: "Players may add or remove spells from their spell book, and may also re-order their spell book, at any time." So players can add spells at any time, provided they have the room in their spell book.
This rule is meant to be a general framework upon which to build, like HP and EXP are. As such, it has a large degree of freedom built into it. Of course, if you don't like something, feel free to propose a change.
As for spell book enlargement, that is something that could be done with spells, items, or not done at all.
